Forms & Letters

Forms and letters are those documents that are outside of the required professional record keeping. Payments for Counseling Services

The payment options that are accepted for counseling services are listed below.
Payment arrangements are established in advance, prior to the initial session.

Private (Self) Pay

You elect to pay for services personally (not using health insurance, EAP, or a third party). The cost of the services, i.e. the full payment (at agreed upon rate) is due from you at the time of service delivery.

Third-Party Payor

You are authorizing submission to a third party (Insurance, EAP program, other party) to cover the use of these services. The practice has agreed to defer costs, depending on your plan you may pay nothing or the co-pay/co-insurance cost as defined in your benefits at the time of service, and your insurance is billed for the service.

Here are some currently accepted Third Party Payors:

Select Health Insurances and EAP programs offered in Illinois, and some out-of-state virtual networks (offer coverage for out-of-state employers with employees in Illinois)

BlueCross BlueShieldof Illinois
BlueCross BlueShieldSelect Virtual Networks
OptumUnitedHealthcare
OptumOscar
OptumOxford Health Plans

Client Portal & Credit Card on File

All clients must have an account on the practice’s client portal platform in order to participate. Failure to obtain an account and/or not having a valid credit card on file will result in the practice not being able to provide services. The practice is currently exploring the best client portal option and uses both Headway and Zencare portal systems.

Portal systems provide connections between medical providers, including those in mental, emotional, and behavioral health services AND various insurance and EAP programs to arrange billing and payments. They also provide secure systems for client documentation and other benefits.

Portal systems are subject to change based on practice needs.
